Requirements
- State issued professional licensed certifications: RN - LPN - CNA - PT - OT - ST - COTA - RTA - CPR - BCLS - ACLS
- Malpractice Insurance
- Current Physical Exam - within one year
- PPD - if negative within one year
- Chest x-ray - if PPD positive within two years
- Proof of lab work required (Measles, Mumps, Rubella, Varicella, Hep B)s
- Passport or other government issued picture ID
- Social Security Card
- INS documentation if non-US citizen
- Proof of Influenza Vaccination
- Reference Letters
- Resume
